CONDUCT & RESOLUTION POLICY
Grievance/Incident Submission Form
Conduct Resolution Procedure
TYH has established this resource to assist parents, coaches, players, and other organizational members in resolving grievances. Within this process, a "grievance" refers to any conflict, dispute, or disagreement among participants, including player parents or guardians, coaches, league officials, or members of different TYH teams, that alleges a violation of TYH Policies & Procedures or USA Hockey Rules and Policies.
Participants are expected to resolve disputes promptly, fairly, and at the lowest possible tier of the Conduct & Resolution Procedure. This process is not designed to address private personal disputes or minor participation concerns, such as playing time, player positioning, or routine team discipline.
All submitted grievances must identify a specific violation of TYH, WAHA, or USA Hockey rules, policies, or procedures. Any allegations involving SafeSport violations must be submitted directly through USA Hockey’s SafeSport Program reporting procedures rather than the Conduct Resolution Procedure.
If you suspect a SafeSport violation has occurred, stop now, consult the SafeSport guidance and reporting options at:
If you suspect a criminal offense, contact law enforcement immediately and report to USA Hockey using the SafeSport report submission tool above.
IF THIS IS NOT A SAFESPORT VIOLATION PLEASE CONTINUE BELOW TO STEPS 1 - 3
STEP ONE:
The 24-Hour "Cooling-Off" Period:
Parents and guardians must refrain from approaching a coach, team representative, or league official to discuss "negative" game or practice situations until at least 24 hours have elapsed following the conclusion of the event.
A confrontation includes any interaction that departs from a calm tone, such as yelling, using profane language, or making disparaging remarks toward officials or staff.
Disregarding this rule constitutes a direct violation of the USA Hockey Zero Tolerance Policy.
STEP TWO:
Informal Resolution:
Grievances concerning a team, its players, or its coaching staff should first be raised verbally with the Team Manager to encourage a collaborative resolution among those involved. Team Managers, coaches, and team officials are expected to handle issues internally in a prompt, fair, and constructive manner.
If a conflict involves TYH members across different teams or occurs at TYH events, it should be brought verbally to the attention of a board member. If these efforts fail to produce a satisfactory outcome, proceed to Step Three.
STEP THREE:
Formal Board Notification:
If the matter remains unresolved after fully completing Steps One and Two, please complete and submit the following form to formally notify the TYH Board of Directors or contact our Conduct & Resolution Manager directly.
